The “Rules Related to Madhya Pradesh State Disease Assistance Fund” scheme was launched by the Department of Public Health and Family Welfare, Government of Madhya Pradesh. The scheme aims to provide financial assistance for major surgical and medical treatments to Below Poverty Line (BPL) patients suffering from life-threatening diseases. > Scheme Implementation: The scheme is implemented by the State Illness Assistance Fund (SIAF) Management Committee, supported by District Collectors, Civil Surgeons, and designated medical institutions.

- For cancer cases, the grant will be given to the JLN Cancer Hospital, Bhopal.

How to Apply

Step 1: The interested applicant downloads the application form from the official website.
Step 2: In the application form, fill in all the mandatory fields, and attach copies of all the mandatory documents (self-attest, if required).
Step 3: Submit the duly filled and signed application form along with the documents to the respective District Collector, addressed to the Director, Public Health and Family Welfare.
