About This Scheme

The “Deendayal Antyodaya Upchar Yojana Card Issuance” scheme has been launched by the Public Health and Family Welfare Department, Government of Madhya Pradesh. The scheme aims to provide free treatment access to Below Poverty Line families within a guaranteed time limit. A Deendayal Antyodaya Treatment Card is issued free of cost to eligible families, including persons with disabilities, to avail government health benefits.

How to Apply

IIn-person

Step 1: The interested applicant downloads the application form (Annexure 1) from the official website.

Step 2: In the application form, fill in all the mandatory fields, and attach copies of all the mandatory documents (self-attest, if required).

Step 3: Submit the duly filled and signed application form along with the documents to the respective designated officer or Lok Seva Kendra.

OOnline

Step 1: The applicant should gather copies of the necessary documents needed to apply for the concerned scheme.

Step 2: The applicant should visit the nearest Common Service Center (CSC) and complete the Biometric Verification Process.

Step 3: The applicant should ensure that the CSC agent completes all mandatory fields in the application form and uploads copies of all required documents.
