Required Documents

  • Proof of Regular Employment and Service Duration.
  • Salary Statement/Income Certificate of the Worker.
  • School Admission Proof.
  • Student's Identity Document.
  • Birth Certificate of the Child (Proof of Age).
  • Disability Certificate of the Student, if applicable, issued by the Medical Officer in the Directorate of Health Service or the Goa Medical College.
  • Caste Certificate, if applicable, issued by the competent authority.
  • Bonafide Certificate from the Institution.
  • Statement from the Worker on the duration of Continuous Service to the Board.
  • Any Other Relevant Documents Specified in the Application Form.

How to Apply

IIn-person

Step 1: The interested applicant should take print of the prescribed format of the application form for the scheme from the Official Website of the Department of Labour and Employment, Goa.

Step 2: In the application form, fill in all the mandatory fields, paste the passport-sized photograph (if required), and attach copies of all the mandatory documents (self-attest, if required).

Step 3: The interested applicant should submit the duly filled and signed application form and the documents through their employers or through the Heads of Educational Institutions where their child is studying, to the Secretary, Goa Labour Welfare Board, 4th floor, Shram Shakti Bhavan, Patto Centre, Panaji, Goa - 403 001.

*The application should reach the Secretary on or before 30th Sept of the current year.
