Conditions

Hospitals must not demand any additional fee from the patient for treatment beyond the sanctioned amount.

The beneficiary or hospital must submit a detailed expenditure report/utilization certificate for the funds spent on the patient's treatment.

A duplicate or photocopy of the discharge ticket issued after the patient's recovery must be provided.

  • Proof Of Being Economically Weaker Section Or Below Poverty Line (Family List/Ration Card).
  • Residence Proof (For Rural Area: Certificate Issued By Chief Executive Officer, Janpad Panchayat; For Urban Area: Certificate Issued By Commissioner, Nagar Palika/Nagar Parishad Official).
  • Recent Color Photograph Of The Patient.
  • Patient's Age Proof (Birth Certificate/Aadhar Card/Panchayat Certificate/Any Other Authorized Document).
  • Detailed Expenditure Report/Utilization Certificate
  • Duplicate Or Photocopy Of The Discharge Ticket Issued After Treatment.
