- ✦Employer’s Certificate ([Form No. 28](https://bocw.punjab.gov.in/bApi/images/file-1703581947529.pdf)) (90 days of construction work during the last 12 months in the State of Punjab). - Nomination Form ([Form No. 27](https://bocw.punjab.gov.in/userfiles/file/Scheme%20Forms/Nomination%20Form.pdf)).

- ✦Life Certificate of beneficiary in the month of January Every Year (Aadhaar based online life certificate through the [Jeevan Pramaan Website](https://jeevanpramaan.gov.in/) which generates digital life certificates may be submitted). - Bank Passbook or Cancel Cheque of Applicant.
