- ✦CBS bank account linked/seeded with Aadhaar card
- ✦Valid family income certificate or BPL certificate
- ✦Disability certificate of child showing more than 40% disability
- ✦Child’s birth certificate
- ✦Copy of family register (for rural areas)
- ✦Copy of the resolution passed in the open meeting of the Gram Panchayat (for rural areas) or by the Councillor (for urban areas)
